Makeup and Hair Consent
THE PAMPER GLAMPER LLC, requires you the Parent to print and sign this waiver. As part of the Spa experience, age appropriate makeup, fun hair extensions and spa treatment products will be applied. This includes, but not limited to lip gloss, eye shadow, hair gel and synthetic hair. We try to do our best to use hypoallergenic, non-toxic products for nails, hair, and make-up, however we cannot promise all items to be safe for your child. All of our makeup and hair applicators are new and disposable or properly cleaned using customary beauty and barber standard cleansing technique & products regulary. I acknowledge that any of the "beauty" or “spa treatments" are for entertainment purposes only and THE PAMPER GLAMPER LLC and all contract workers working with THE PAMPER GLAMPER LLC are not licensed cosmetology professionals. THE PAMPER GLAMPER LLC will not cut, file, trim, push back cuticles, wash or style hair or do any services provided at a certified nail or hair salon. Nails should come polish free, free of debris as well as hair should come clean and styled. THE PAMPER GLAMPER LLC is an event planning company and the spa party themes are for entertainment purposes only.
Photo Release Form for Minors
I, the parent or legal guardian of the child/children mentioned below grant THE PAMPER GLAMPER LLC, my permission to use my child’s likeness, image, voice, and/ or appearances as such that may be embodied in any pictures, photos, video recordings, digital images, and the like, taken or made on behalf of THE PAMPER GLAMPER LLC, party activities. I agree that THE PAMPER GLAMPER LLC. has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with THE PAMPER GLAMPER LLC Mobile Spa Parties. These uses include, but are not limited to, reprints, publications, advertisements, social media and any promotional materials in any medium now known or later developed, including the internet. I acknowledge that I will not receive any compensation, etc for the use of such pictures, etc., and hereby release THE PAMPER GLAMPER LLC, from any and all claims which are in any way connected with such use. I have read and understood this consent and release.
Food Allergy Waiver
Your child will be participating in an entertaining spa like experience with craft and beauty activities. Some of these activities involve food and facial products as part of the experience. We know that many children have allergies and some of these allergies can be life threatening. We take many precautions to ensure that all the children are safe. THE PAMPER GLAMPER LLC will not be held responsible or liable for any allergic reaction or life-threatening incident including death that may occur. If your child has any known food allergies or intolerances that will prevent her/him from participating in these activities, we will provide an alternate option at your request.
Coronavirus/COVID-19
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.
I further acknowledge that THE PAMPER GLAMPER LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.
I further acknowledge that THE PAMPER GLAMPER LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Party staff, and other guest and their families.
I voluntarily seek services provided by THE PAMPER GLAMPER LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I and my family must comply with all set procedures to reduce the spread while attending an event with THE PAMPER GLAMPER LLC
LIABILITY
We Strictly provide entertainment. Please be aware that children need to be supervised. We cannot be held responsible for accidental damage to themselves or other and/or the vehicles/ RV involved caused by your child. THE PAMPER GLAMPER LLC assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss or our inventory and/or damage or loss to the party RV, the responsibility lies entirely on the client and or parent/guardian. THE PAMPER GLAMPER LLC is not responsible for allergic reactions and / or any medical injuries from the food, beverages, wearing costumes, hair products etc. or any activity taking place before, during or after the party.
I hereby release and agree to hold THE PAMPER GLAMPER LCC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or that may otherwise arise in any way in connection with any services received from THE PAMPER GLAMPER LLC. I understand that this release discharges THE PAMPER GLAMPER LLC from any liability or claim that I, my family, or any personal representatives may have against the business with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from THE PAMPER GLAMPER LLC This liability waiver and release extends to THE PAMPER GLAMPER LLC together with all owners, partners, and employees.
